Cancellations

I don't normally ask for deposit other than if any of the situations below applies...

1)If you cancel less than an hour prior to your
appointment time.
2)If you have been a no call/no show, (NC/NS)
before.
3)If the agreed amount is over $500.
4)If I'm driving 20miles or more to get to the
appointment.
